This guide sticks to what is actually understood: how the chamber works, which benefits have decent evidence behind them, which are still speculative, and what the therapy will not do no matter how many sessions you book.

What is hyperbaric oxygen therapy?

Hyperbaric oxygen therapy, usually shortened to HBOT, involves breathing concentrated oxygen inside a chamber where the air pressure is raised above the level you normally live at.

Sea level pressure is 1 atmosphere absolute, written as 1 ATA. A hyperbaric chamber typically runs somewhere between 1.3 and 3.0 ATA, depending on what it is being used for. Wellness and recovery settings tend to sit at the gentler end, around 1.3 to 1.5 ATA. Hospital protocols for serious conditions go considerably higher.

The pressure is the part people miss. Breathing pure oxygen at normal pressure does very little, because the haemoglobin in your red blood cells is already close to fully saturated. Pressure is what changes the equation.

How does hyperbaric oxygen therapy work?

Under normal conditions, almost all the oxygen you carry is bound to haemoglobin. Only a small fraction dissolves directly in your blood plasma, and that fraction is largely irrelevant.

Raise the pressure, and that changes. Henry’s law says the amount of a gas that dissolves in a liquid rises with pressure, so pressurising the chamber forces far more oxygen into solution in the plasma itself. That dissolved oxygen behaves differently from the haemoglobin-bound kind. It is not dependent on red blood cells reaching a tissue, so it can diffuse into areas where circulation is poor, swollen or partially blocked.

That is the whole mechanism in one sentence: HBOT gets oxygen to places blood is struggling to reach.

From there, a few things are thought to follow. Higher tissue oxygen supports the collagen production and cell division involved in repair. It appears to encourage the growth of new small blood vessels over a course of sessions, a process called angiogenesis. It affects inflammatory signalling. And oxygen at high concentration is directly hostile to certain bacteria that thrive in low-oxygen tissue.

It is worth separating the clinical evidence from the wellness marketing, because they are not the same thing.

Where the evidence is strongest

HBOT has an established, well-documented role in a defined set of medical conditions. Decompression sickness in divers is the original one. Carbon monoxide poisoning is another. It is also used in hospital settings for certain non-healing wounds, most notably diabetic foot ulcers, and for tissue damage caused by radiotherapy. Chronic wounds affect millions of people in the UK and cost the NHS billions a year, which is why the wound healing research base is the deepest part of the field.

Where the evidence is promising but not settled

A second tier of uses has reasonable mechanistic logic and some supportive research, but nothing you could call conclusive.

Recovery from hard training sits here. The theory is straightforward, since intense exercise creates micro-damage and localised inflammation, and more oxygen to the tissue should, in principle, speed repair. Some studies in athletes report reduced soreness and faster return to performance. Others find no meaningful difference against a placebo chamber. The honest summary is that it may help, and the effect size is probably modest.

Post-surgical recovery, persistent fatigue, sleep quality and cognitive fog all fall into the same category. Studies are pointing in encouraging directions, particularly around fatigue and cognitive symptoms, but the trials tend to be small, and the placebo problem in this field is genuinely difficult to solve. Sitting in a chamber for an hour is not a neutral experience.

Where claims outrun the evidence

You will see HBOT sold as an anti-ageing intervention, a treatment for a long list of chronic conditions, and occasionally as something close to a cure-all. Be sceptical. The gap between “oxygen supports cellular repair” and “this will reverse your biological age” is enormous, and most of what bridges it is marketing rather than data.

What does a session actually feel like?

Undramatic, mostly. You sit or lie inside the chamber, which is either a rigid shell or a soft-sided inflatable design, and the door is sealed.

Pressure then rises gradually over several minutes. This is the only part most people notice, because your ears need to equalise in exactly the way they do on a descending aircraft. Yawning, swallowing, or a gentle Valsalva manoeuvre sorts it out. Say something if it becomes uncomfortable, because the pressurisation can simply be slowed down.

Once you are under pressure, you breathe the oxygen, usually through a mask or hood. Sessions commonly run 60 to 90 minutes. There is nothing to do. People read, listen to something, or fall asleep.

Decompression at the end is another slow, gradual few minutes. Afterwards, some people feel noticeably energised, and others feel pleasantly tired. Both are normal.

How many sessions do you need?

There is no single answer, and anyone who gives you one without asking why you are there is guessing.

Clinical protocols for wound healing typically involve courses of twenty to forty sessions, run several times a week. That volume is what the angiogenesis research is built on, because new blood vessel growth is a cumulative process rather than something that happens in an afternoon.

For general recovery and wellbeing purposes, people tend to use it more sparingly, often as a block of sessions around a heavy training period or a recovery window, then intermittently after that. A single session is a reasonable way to find out whether you like the experience. It is not a fair test of whether the therapy does anything for you.

What are the side effects of hyperbaric oxygen therapy?

Most people will tolerate it, but it is not risk-free, and it is worth knowing what you are agreeing to.

The most common issue by a distance is ear barotrauma: pressure-related discomfort or, less often, injury to the eardrum. This is almost entirely a function of how well you equalise during pressurisation, which is why the ramp is slow and why it’s important to speak up rather than tough it out.

Sinus discomfort can happen for the same reason, particularly if you are congested. Some people feel tired afterwards. Temporary vision changes, usually a mild short-sightedness, are reported with long courses of treatment and typically resolve over the weeks after the course ends.

Rarer effects include claustrophobia in the chamber, which is worth flagging in advance if enclosed spaces are difficult for you, and oxygen toxicity, which is a genuine risk at high pressures and long exposures and is the reason session length and pressure are controlled rather than maximised.

Who should avoid it?

HBOT is not appropriate for everyone. An untreated pneumothorax, a collapsed lung, is an absolute contraindication. Certain lung conditions, some ear and sinus problems, recent ear surgery, and specific chemotherapy drugs are all reasons to be assessed properly first.

Pregnancy, uncontrolled high fever, a history of seizures and some cardiac conditions all warrant a conversation with a doctor before you book anything. Any provider worth using will screen you for these. If nobody asks, that tells you something about the provider.

What hyperbaric oxygen therapy will not do

It will not replace sleep, training load management or eating properly. Recovery interventions are a multiplier on the basics, not a substitute for them.

It will not fix an injury that needs a physiotherapist or a surgeon. It will not deliver a dramatic change from one session, and it is not a treatment for conditions where the evidence does not support it, whatever a chamber operator tells you.

So is it worth it?

If you are recovering from surgery, working through a stubborn wound, carrying persistent fatigue, or training hard enough that recovery has become the limiting factor, HBOT is a reasonable thing to try. The mechanism is real, the safety profile is good when it is run properly, and the downside of a course of sessions is mostly time and money rather than risk.

If you are expecting it to transform your health on its own, you will be disappointed. It works best as one input among several, alongside sleep, training, nutrition and everything else that is less interesting to write about.
